Correlation of Leukoplakia with Velscope Delineated Margins and Histopathology

Sixty year old gentleman without history of smoking and 3 previous excisions of mod-severe dysplasia. Recurrence of the leukoplakic lesion prompted referral to OSU. The continuous line made with a tissue marker represents the surgeon's outline of what he perceived to be lesional tissue. The dotted outline of the mucosa represents reduced fluorescence by VELscope. Looking very closely, there may be a hint of increased vascularity in the "positive" (dotted) region outside the surgeon's line. But the surface epithelium looked totally normal. The patient was aware of VELscope from the internet and requested that the surgical margin include all "positive" mucosa. Processing and specimen orientation was carefully recorded to permit assessment of the entire specimen. The following are the corresponding images.
